Your privacy is important, all information collected is confidential & will not be shared. Some medical information is collected to ensure your safety during any & all procedures being preformed. Please ensure this information is accurate & up to date with any recurring appointments.
Personal information like medical info, email, phone number etc. will be kept in your confidential file, along with additional details pertaining to your appointments, such as: pigments used, any sensitivities, notes for touch up, & updates you provide through the healing process, etc.
A Photo/Video Consent form will be included in your package to allow your procedure to be shared through before & after photos & on rare occasion a video. Any videos being taken will be verbally discussed before hand, in addition to the consent form. These photos/videos are strictly used for marketing purposes.
Please Note credit card & banking information is NOT stored in your personal file.
